Demand Management (leading to pathway redesign)

As soon as Practices get comfortable working with their data we can then go up a level to sampling data with a view to altering our demand management. We suggest you look to include this within the Incentive Scheme Plan as a basic level start with PBC.

As part of a review of overall demand it is normally a requirement that Practices sample the data from selected specialties, and provide a number of first attendance referral letters, along with the discharge summary, or hospital letter to advise next treatment steps. Additionally, we require the same for follow up appointments; these should be accompanied by the latest contact letters confirming why patients continue to attend the department.
